This project created a secure, integrated health data platform that connects information from general practice and hospitals across Western Sydney, to help clinicians can gain a more complete view of patient care journeys and make better-informed decisions to improve health outcomes.
Opportunity: In 2023 the Western Australia Country Health Service (WACHS) implemented the WA Antimicrobial Stewardship digital decision support tool known as Antimicrobial Pathways (AMPs) to support improved antimicrobial prescribing across regional and remote healthcare settings. The project set out to provide evidence on the impact of AMPs on improving prescribing quality, adherence to antimicrobial guidelines and strengthening antimicrobial stewardship practices across the health system.
Opportunity: This project evaluated the Alfred Health’s COVID Community Care Pathway Program to identify lessons that could shape future virtual care programs and pandemic preparedness initiatives.
