Project objective
The Alfred Health COVID Community Care Pathway Program was stood up very quickly in 2020 to monitor COVID patients in the community. The program involved a “light touch” approach to digital health, with a pulse oximeter, thermometer, and daily ratings initiated by text messages sent by a custom-built digital platform. The platform monitored symptoms which were triaged for nursing or medical escalation.
The project team will formally assess the program with a view to describing the digital health and model of care learnings and apply these to other remote models of care and potential pandemic situations in the future.
