Internship project: Chronic Conditions Care Coordination Service evaluation

Opportunity

Patients with chronic conditions have a high number of unplanned hospital presentations and inpatient admissions and readmissions. This project will evaluate the impact of an integrated care approach in rural WA and explore best practices in the literature for rural and remote chronic condition care coordination services. The Chronic Conditions Care Coordination Service (CCCCS) within the WA Country Health Service is run by WACHS central service but each region has independent and varied services. The project aims to describe each service and then complete a literature review of similar and relevant services. This service description and literature review will form the basis of a larger project and be included in a proposal for grant funds.

Project Objective

The main focus of the internship is an in-depth and systematic literature review with the aim of identifying, structuring and visually mapping the current CCCCS existing academic and practitioner literature related to measures for value-based (and quality improvement oriented) healthcare.
