Project objective

This project involved an independent evaluation of a virtual home care program for recently discharged patients with complex cardiac conditions. The project team implemented and assessed a virtual home care pilot at Alfred Health, allowing for continuity of care for patients through the usage of home monitoring devices. Using the pilot as a study, the project team reported on the implementation of the program and provided recommendations on next steps for the implementation of virtual home care programs in hospitals.
