Opportunity 

Health systems are increasingly seeking safe and effective alternatives to hospital-based care for patients with chronic and complex conditions. Alfred Health identified an opportunity to support patients with heart failure through a virtual home care model using remote patient monitoring, helping patients remain at home while maintaining clinical oversight.  

This project evaluated the implementation of a virtual home care program at Alfred Health for patients with complex cardiac conditions.  

Project Objectives  

This project involved an independent evaluation of a virtual home care program for recently discharged patients with complex cardiac conditions that allowed for continuity of care through the use of remote monitoring devices.  

The project team reported on the feasibility, safety, patient adherence and user experience of the program, and provided recommendations on the broader deployment of remote monitoring services to support the implementation of virtual care programs across health services. 

The pilot demonstrated that remote monitoring of patients with complex cardiac conditions is feasible, safe and acceptable to patients. Participants showed strong adherence to physiological monitoring and reported benefits including improved confidence, health literacy and self-management. The evaluation also identified practical implementation challenges, particularly technology integration, technical support requirements and alert management protocols, providing valuable guidance for future service design. 

The project delivered important evidence supporting digitally enabled hospital-at-home care models in Australia. The findings provide a foundation for scaling virtual care services, supporting more patient-centred care pathways and helping health services improve access, efficiency and chronic disease management. The implementation insights developed through the project may be applicable across a range of future remote monitoring programs and patient cohorts.
