Evaluate the impact of the Alcidion electronic patient journey board across Alfred Health sites.

Project objective

Patient journey boards are a visual management tool used to provide access to critical information about each patient’s current progress such as active referrals, discharge plan and discharging barriers for facilitating timely discharge.

The electronic patient journey board will be implemented across Alfred Health to support progression of care along with clinical and operational decision making. Electronic patient journey boards will display a combination of integrated and manually updated data, replacing non-electronic whiteboards.

The project team will evaluate the impact and effectiveness of the electronic patient journey boards across Alfred Health sites.
